Standard E12: Elderly & palliative health care services are provided as per guidelines

  • The standard measures adequacy & quality of elderly & palliative care provided by HWC (SC).

  • Standard include assessment of process to ensure screening of palliative cases, arrangement of home care visits as per patient’s need, management of pain, bed sores and availability of established system for provision of care to terminal cases etc.

  • Staff and patient party should be interviewed & record should be reviewed for holistic assessment.

Measurable Elements

  • ME E12.1: The facility ensures primary assessment, screening, and identification of elderly patients and individuals requiring palliative care in the catchment area.

    (Focuses on identifying bedridden, terminally ill, chronically frail, and elderly individuals through community survey/CBAC and maintaining line-lists).

  • ME E12.2: The facility ensures provision of primary clinical care, pain management, and supportive care for elderly and palliative patients at the facility.

    (Focuses on managing age-related health conditions, basic pain relief, management of bedsores, providing assistive devices, and counseling for patients and family caregivers).

  • ME E12.3: The facility conducts home care visits and coordinates community-level follow-up care for bedridden, elderly, and palliative care patients.

    (Focuses on organized home visits by HWC team/ASHAs/MPWs, caregiver training, supply of essential consumables/dressings, and ensuring continuity of home-based support).

  • ME E12.4: The facility ensures effective referral, tele-consultation support, and linkages with higher health facilities for specialized elderly and palliative care.

    (Focuses on coordinating with secondary/tertiary palliative centers, facilitating tele-consultations (eSanjeevani), and managing emergency care/referral for severe end-of-life complications).
