  • Assessor should review these records to ensure that theses indictors are getting measured at HWC (SC) regularly.

  • ME H1.1: The facility measures productivity indicators related to outpatient care and service utilization.

    (Focuses on regular recording and monitoring of daily OPD attendance—including new vs. old cases—and coverage of specialized clinics).

  • ME H1.2: The facility tracks coverage of target populations and program-specific outcomes.

    (Focuses on measuring progress against enumerated target populations for screenings, active cases initiated on treatment, routine immunization, and maternal/child health milestones).
