Ensuring provision of the expanded scope (12 comprehensive primary healthcare services) of primary healthcare under the Health and Wellness Centre (Sub-Centre) framework, to span the entire continuum of care—encompassing preventive, promotive, curative, rehabilitative and palliative aspects to their catchment population to cover full life-course care.
Services Provision measures availability of functional services in HWC-SC.
True service availability goes beyond merely having infrastructure and human resources in place. A service is only considered truly functional when it is actively utilized by the community and ensures a complete cycle of care includes regular diagnostic testing, consistent medication distribution, and ongoing patient assessments.
Standards are verified by observing service delivery mechanisms, reviewing facility records, and tracking actual health outcomes post-delivery.
