• concerned with formulation and assessing functionality of Primary health care quality team at facility.

  

• Assessor may review the document and interact with team members to know about their roles and responsibilities and how frequently they meet and review their functioning records.

• ME G1.1: The facility has a functional Quality Team/Committee in place for Quality Improvement.

  (Focuses on formal constitution of the Primary Health Care Quality Team at the facility, clear designation of roles and responsibilities among staff, and regular conduct of quality meetings).

• ME G1.2: The facility regularly monitors quality indicators and tracks progress on Quality Improvement (QI) projects.

  (Focuses on maintaining meeting records, reviewing key performance/quality metrics, identifying gaps, and executing Plan-Do-Check-Act (PDCA) cycles or corrective actions).
