Standard E6: The facility has defined and established procedure for nursing care
standard measure adequacy & quality of patient care provided in HWC (SC).
It includes processes for identification of patients, timely and accurate implementation of the prescribed treatment plan, maintenance of records and monitoring of the patients as directed by referral centre.
The staff should be interviewed, and patients’ records should be reviewed for assessing how drug distribution takes place, how its administration is ensured and its record, and other procedures like sample collection and dressing have been done on time as per treatment plan.
ME E6.1: The facility ensures standard procedures for patient identification, vital sign recording, and initial nursing assessment.
(Focuses on correct identification of patients, baseline measuring of temperature, pulse, blood pressure, respiratory rate, and initial screening protocols).
ME E6.2: The facility ensures timely and accurate implementation of prescribed treatment plans and clinical procedures.
(Focuses on carrying out medical orders on time, maintaining dressing/wound care standards, nebulization, minor surgical support, and tracking procedural progress).
ME E6.3: The facility maintains complete records and documentation of clinical/nursing care provided to patients.
(Focuses on documenting vital logs, treatment delivery sheets, procedure notes, dressing logs, and nursing observations accurately).
ME E6.4: The facility ensures patient monitoring during and after procedures, as well as adherence to referral instructions.
(Focuses on post-procedure monitoring, observing for adverse events or complications, and following treatment instructions provided by referral centers).
