Standard H3: The facility measures clinical care indicators

• The standard is concerned with clinical Quality such as early identification, correct diagnosis, initiation of treatment, Chronic illness patient with treatment compliance, follow up of the cases under treatment, timely referral at appropriate facility etc.

• Record review can be done to see the measurement of these indicators.

• ME H3.1: The facility measures clinical care indicators on a regular/monthly basis.

  (Focuses on tracking key clinical metrics across health programs—such as percentage of high-risk pregnancies identified, blood pressure and glycemic control rates in NCD patients, TB treatment completion rates, and AEFI cases reported).

• ME H3.2: The facility monitors quality of clinical processes, follow-up, and referral outcomes.

  (Focuses on reviewing patient treatment compliance, tracking post-referral updates for critical or chronic cases, and utilizing record reviews to ensure early diagnosis and appropriate initiation of care).
